Medicaid is a health coverage program for Utah residents with limited income, people with disabilities, seniors, and children who need medical support. You can apply any time of the year.

Fill out the Medicaid application (it takes about 20–30 minutes).
Upload or mail any documents DWS (Department of Workforce Services) asks for — like proof of income or ID.
Wait for your approval notice. Once you’re approved, you can apply for one or more of the waivers listed below.
Who it’s for: Children and adults with intellectual or developmental disabilities.
Example: Autism, cerebral palsy, Down syndrome, or other lifelong developmental conditions.

Complete their intake form and upload any medical or school evaluations.
DSPD will review eligibility and place you on the waiting list.

Services Include:
Day programs, respite, in-home supports, employment help, transportation, and behavior services.

How to Apply:

Specify that you’re applying for the ABI Waiver.
Provide medical records showing the brain injury and current support needs.
Once accepted, you’ll be added to the waiting list.
Services Include:
Neurotherapy, case management, personal assistance, and supported employment.
Who it’s for: Adults with serious physical disabilities who need daily assistance.
Requirements:
Must have loss of function in at least two limbs.
Must need 14+ hours/week of personal assistance.
Must be able to direct your own care (or have someone who can).
Apply Through DSPD: dspd.utah.gov.
Services Include: Personal assistance, case management, environmental adaptations, and respite.
Who it’s for: People currently living in an intermediate care facility (ICF) who want to move into the community.
Steps to Apply:
Contact your facility’s social worker or DSPD.
Ask to apply for the Community Transitions Waiver.
A DSPD transition manager will help create a discharge and support plan.

Who it’s for: Adults leaving a nursing home or assisted-living facility.
Steps:
Confirm that you’ve lived in a qualifying facility long enough (90 days for nursing home, 365 for assisted living).
Apply online at Utah New Choices Waiver.
Or call 801-538-6155 (Option 6) for help.
Services Include: Housing assistance, case management, homemaking, respite, and transportation.

Who it’s for: Seniors who need nursing-home-level care but want to stay home.

Waitlist placement is based on the child’s medical needs.

Services Include: Case management, in-home nursing, equipment, and respite care.
Who it’s for: Children under 21 who rely on ventilators, feeding tubes, or other life-support technology.

You’ll need medical documentation from your child’s doctors.
Approved applicants are placed on a waitlist based on medical urgency.
Who it’s for: People with developmental disabilities or brain injuries who need smaller amounts of support.
How to Apply:
Apply to DSPD first.
Ask your DSPD worker to review if you qualify for the Limited Supports Waiver.
Services Include: A flexible yearly budget for self-directed services and goods (usually around $21,000 per year).
