DSPD Medicaid Waiver Services

The Division of Services for People with Disabilities (DSPD) offers Medicaid Home & Community-Based Services (HCBS) waivers that support individuals with disabilities in living fulfilling lives at home and in their communities.

DSPD waivers are designed to provide person-centered supports that promote independence, dignity, safety, and meaningful participation in everyday life — without requiring placement in an institutional setting.

Rather than a one-size-fits-all approach, services are tailored to each individual’s needs, goals, and strengths.

How to apply:

Step 2: Eligibility Review

A DSPD intake worker will complete an assessment to evaluate:

  • Disability category

  • Functional limitations

  • Level-of-care needs

  • Medicaid financial eligibility

Step 3: Documentation

Upload or mail any documents DWS (Department of Workforce Services) asks for — like proof of income or ID.

Step 3: Waiting List Placement

If eligible, the individual is placed on the statewide waiting list until waiver funding becomes available.

 

DSPD Waivers

Community Supports Waiver (CSW)

Who it’s for: Children and adults with intellectual or developmental disabilities. Click Here for CSW fact Sheet 
Example: Autism, cerebral palsy, Down syndrome, or other lifelong developmental conditions.

How to Apply:

  1. Contact DSPD (Division of Services for People with Disabilities): Apply Here.

  2. Complete their intake form and upload any medical or school evaluations.

  3. DSPD will review eligibility and place you on the waiting list.

  4. Keep your contact info current; they’ll reach out when funding is available.

Services Include:

  • Day programs, respite, in-home supports, employment help, transportation, and behavior services.

Acquired Brain Injury (ABI) Waiver

How to Apply:

  1. Contact DSPD Intake (1-844-275-3773)

  2. Specify that you’re applying for the ABI Waiver.

  3. Provide medical records showing the brain injury and current support needs.

  4. Once accepted, you’ll be added to the waiting list.

Services Include:

  • Neurotherapy, case management, personal assistance, and supported employment.

 

Physical Disabilities (PD) Waiver

Who it’s for: Adults with serious physical disabilities who need daily assistance. Click Here for the ABI fact sheet.

Requirements:

  • Must have loss of function in at least two limbs.

  • Must need 14+ hours/week of personal assistance.

  • Must be able to direct your own care (or have someone who can).

Apply Through DSPD: dspd.utah.gov.

Services Include: Personal assistance, case management, environmental adaptations, and respite.

Community Transitions Waiver (CTW)

Who it’s for: People currently living in an intermediate care facility (ICF) who want to move into the community.

Steps to Apply:

  1. Contact your facility’s social worker or DSPD.

  2. Ask to apply for the Community Transitions Waiver.

  3. A DSPD transition manager will help create a discharge and support plan.

Limited Supports Waiver (LSW)

Who it’s for: People with developmental disabilities or brain injuries who need smaller amounts of support. Click here for the LSW fact sheet.   
How to Apply:

  1. Apply to DSPD first.

  2. Ask your DSPD worker to review if you qualify for the Limited Supports Waiver.

Services Include: A flexible yearly budget for self-directed services and goods (usually around $21,000 per year).
