EPAS Medicaid & Case Management Services
The Employment Personal Assistance Services (EPAS) program through Utah Medicaid helps individuals with disabilities stay independent by providing support for daily living and employment needs. At CREJ, we work as your service coordinator to guide you through the EPAS intake process, ensuring you have access to all the services based on your assessed needs. Steps to Enroll with CREJ EPAS Case management:
Step 2: Proxy Agency Information
Under the section "Do you have a proxy or agency to help you with the EPAS application process?" Put this information:

Step 3: Freedom of choice form
When filling out the freedom of choice form, select CREJ LLC as your provider of choice.
Step 4:
Once CREJ LLC receives your referral you will be contacted within 24 hours
Through EPAS, we can help you access personal assistants for:
- Mobility in and around your home or workplace
- Meal preparation and feeding assistance
- Personal hygiene and grooming
- Dressing and transferring
- Shopping and errands
- Bathing and laundry
- Ordinary housework
- Managing finances and budgeting
- Medication reminders
- Transportation for work
